In order to provide our service to you, Freed Beyond collects and uses some of your personal information. Protecting your personal data is important to us. We are committed to complying with all data protection legislation, including the Data Protection Act 2018 and the UK General Data Protection Regulation (UK GDPR)

What Information do we Collect?

Freed Beyond will only ask you for, and hold, the personal information that is necessary to provide the services you require.

This includes:

  • Your name and contact information provided to us at referral
  • Date of birth
  • Doctor’s details
  • General demographic details – gender, age, ethnicity etc
  • Next of Kin details in case of an emergency
  • Record of sessions
  • Relevant history
  • Records of any correspondence

Lawful Basis for Holding your Information

Freed Beyond collects and uses your personal data under the valid reason (i.e. the ‘lawful basis’) of Consent.

You can refuse consent but if you do not provide the information required, we may be unable to provide the services you have requested.

What we do with it

We obtain your personal information in order to contact you, to enable us to provide our service to you and to keep you safe while we are working with you, in line with our safeguarding and confidentiality policies.

Your information will be stored electronically on a secure cloud-based system; any hard copy documents are secured in lockable filing cabinets.

Who do we share your data with?

All personal data we hold about you will be used only by those requiring access i.e. therapist, administration staff.

We will not normally share your data with anyone else. However, in certain circumstances, where it is essential for your wellbeing and safety we may have to share your personal information with [e.g. other medical professionals]. We will seek your permission before doing so, unless in an emergency situation where this would not be possible.

Anonymised statistical data will be used for evaluation and reporting purposes.

How long we keep it

We will hold your information for no longer than 7 years, when it will be confidentially destroyed.

Your Rights

You have the following information rights:

  • The right of access. You can ask for a copy of the data we hold on you; this is known as a subject access request (SAR).
  • The right to object. This means that you can withdraw your consent or object to specific processing of your personal data. This may mean that we would be unable to provide the services you requested.
  • The right to be informed about your personal data that we hold and what we are doing with it.
  • The right to rectification. This means that you can ask us to correct your data if it is not accurate.
  • The right to erasure. This means that in certain circumstances you can ask us to delete your data.
  • The right to restrict processing. This means that you can ask us to temporarily stop processing your data if you ask us to. We could store your data, but not use it. This is not an absolute right and only applies in certain circumstances.
  • The right to data portability.
